Patient Medical Record Request Process

To request medical records you will need to contact a release of information associate between the hours of 8:00am and 5:00pm, Monday through Friday, at (502) 629-8766. Associates will be able to assist you with the request process. By calling ahead and speaking to a release of information associate, you may avoid having to wait while records are copied and/or printed, or avoid an unnecessary trip to one of our hospitals. We can not guarantee that your request can be processed while you wait if you present a request in person and have not called ahead.

Records needed for continuing care purposes will be sent directly to the provider eliminating the need for you to make a trip to Norton Healthcare.

Requests for medical records for reasons other than continuing care, or for someone other than yourself, must be processed in person. We require a signed authorization and a valid photo ID. You can obtain an authorization form from any of the Norton Hospital HIM departments or by clicking on the Medical Record Request/Authorization Form. These requests are processed at the 210 East Gray Street location but can be picked up at any of the Norton Hospital Health Information Management departments.

Medical record information will not be released over the telephone.

If you need records from Norton Physician Practices, ICCs, or CMA offices you will need to contact those sites.

If you need films from Radiology procedures you will need to contact the appropriate Radiology Department or Diagnostic Center:
